• $250.00 check payable to the Sheriff of Chester County.
• Properly completed Chester County Sheriff’s Return Form indicating instructions to “LEVY & MAIL” – (see #8 on Service Form)
• Two (2) time stamped Writs of Execution (one (1) for mailing to defendant, one (1) for return)
• One (1) Claim for Exemption Form (sample in PA. Rules of Court)
• One (1) Writ of Execution Notice(sample in PA. Rules of Court)
• One (1) postage paid envelope addressed to defendant
• One (1) postage paid envelope addressed to attorney on writ

If defendant is a corporation or company, it is not necessary to mail unless requested by the attorney.  The above requirements are for one (1) defendant.  For each additional defendant, enclose one (1) time stamped Writ, one (1) Notice, one (1) Claim Form and one (1) postage paid addressed envelope in addition to the requirements stated above.

• $150.00 check payable to the Sheriff of Chester County.
• Properly completed Chester County Sheriff’s Return Form indicating instructions to “GARNISH” – (see #8 on Service Form)
• Two (2) time stamped Writs of Execution (one (1) for defendant, one (1) for garnishee)
• One (1) Claim for Exemption Form (sample in PA. Rules of Court)
• One (1) Writ of Execution Notice (sample in PA. Rules of Court)
• One (1) postage paid envelope addressed to defendant
• One (1)  postage paid envelope addressed to attorney on writ
• Three (3) copies of Interrogatories (optional)
