Sewage/Well File Lookup Form
This form requires the download of Formatta Filler. This is a very easy process and only takes a few seconds to download. There is no fee associated with this download.
1. Download Formatta Filler - Please note when the registration page appears after the download is complete, it is not necessary to fill out the registration form, just close the window. You do not need to register Formatta Filler.
3. Fill in the information on your computer. You cannot print the form and fill it in by hand. Click the Print button at the bottom of the form when you are finished filling it out.
