Life at Home
 
LIFE at Home is the local subsidiary of the PACE Model program, PACE as in the Federal Program of All-Inclusive Care for the Elderly, not the PACE prescription program. LIFE at Home became an available service to Chester County residents on July 1, 2007. The program is designed to be a “one-stop shop” for persons to have access to such services all in one location.
 
Services may include:
 
Physician and nursing care
Prescriptions
Physical therapy
Dental, eye & foot care
Meals
Adult day care
Personal care
 
In addition, consumers in the program may be able to access the same services at home, even other assistance such as in-home care, medical equipment and supplies, and home support or chore service.Services are provided under Medicare and/or Medicaid. If a participant is not eligible for
Medicaid, he or she may be required to cost share for services.
 
Qualifications for the program include:
 
Age 55 or older
Chester or Delaware County resident
Physically require a nursing home level of care
 
LIFE at Home is located in Sharon Hill, Delaware County, but Chester County residentsare able to attend this facility, with transportation provided independently by the program. At this time there are no contracted providers within Chester County, however it is anticipated that as the program grows, we will have local providers.
